The Radiotherapy Department at Gloucestershire Oncology Centre has launched an advice line for patients, carers and health care professionals. This is an answer machine service for GPs and clinical teams to call with any queries about any side effects patients are experiencing. Messages are picked up daily and the service will call back within two working days. All patients are given the telephone number at the end of their radiotherapy treatment to use should they have any questions or concerns. |

The late effects of radiotherapy (developing six months or longer post-treatment) can vary widely and, depending on the part of the body treated, may include bowel and bladder disorders, pain, swelling, sexual dysfunction and fatigue. These physical symptoms can develop months, years, or even decades after undergoing radiotherapy, and can have a significant impact on a patient’s quality of life, often leading to emotional distress. Late effects can develop after many years and patients therefore don’t always associate their symptoms with their previous cancer treatment. They can include chronic pain, incontinence and lymphoedema or swelling in the area, skin changes, and loss of movement or function. For example, if the pelvis was treated there may be bowel, bladder, gynaecological or erectile symptoms. Whichever part of the body radiotherapy was delivered to, there may be intimacy and sexual wellbeing concerns. The service provides specialist advice for patients with long-term complications of radiotherapy. It offers a range of strategies including assessment of symptoms, patient information, treatment care plans and medications to support people to live well with the late effects of radiotherapy. Onward referrals to other healthcare professionals are also made where appropriate. We hope to reduce the impact late effects are having and improve quality of life – this may be by reducing pain, improving bowel or bladder habits, and improving function. It is important to realise that it is not always possible to remove radiotherapy late effects altogether. |

The service is aimed at anyone from Gloucestershire who has completed radiotherapy treatment more than six months ago and is now experiencing difficulties that may be related to their treatment. Initially, patients can be referred to the Late Effects service by GPs and other healthcare professionals and they will soon be able to self-refer. It is important that any cancer recurrence or new pathology is ruled out prior to referral as appropriate. |

NHS England is working with EUROPAC to provide a route from the NHS into their national surveillance program run at NHS Trusts across the country for people who may be at high risk of pancreatic cancer. The European Registry of Hereditary Pancreatic Diseases (EUROPAC) is in the first instance a registry for families with histories of Familial Pancreatic Cancer and Hereditary Pancreatitis. We register these individuals and collect their family history as well as samples for research purposes. The EUROPAC study also runs surveillance programs for individuals who are deemed high-risk. Using the family history individuals provide we can assess their lifetime risk of developing pancreatic cancer. Surveillance is offered on a yearly basis, and we use a combination of CT, EUS, MRI and blood tests. |

The cardiac rehabilitation programmes are medically supervised programmes of exercise, emotional support, diet and exercise that can help improve quality of life and help manage a condition. The cardiac rehabilitation service is made up of skilled specialist nurses, and works in partnership with a range of other organisations, including national research and charitable organisations such as the British Heart Foundation. Nurses provide a home visiting service to patients who have had a heart attack or coronary artery bypass graft surgery. We accept patients following CABG, STEMI/NSTEMI or PCI for Phase II and III cardiac rehabilitation. Patients following Valve surgery are accepted for Phase II telephone assessment only. Other patients are accepted on an individual basis e.g. following Heart Transplant. Referrals are received from Consultants/Ward Nurses/ GPs / PNs/ other CR centres and Physiotherapists. Referrals may also be made to our email address or by phone. Patients should be offered a service nearest to their home therefore we will accept patients who are registered with a GP in a neighbouring county. Similarly we may refer on to a more local service to the patient. Patients receive Phase II assessment call within 4 days of discharge and are then either directly recruited onto a Phase III programme (elective PCIs) or a Phase III home visit is arranged for further assessment. |

Gloucestershire Fire and Rescue Service has a dedicated team of operational firefighters and Community Safety Advisors (CSA's) that conduct 'Safe and Well' visits at people's homes throughout Gloucestershire.The CSA's have received additional training to give people in their homes not only fire safety advice, but advice on their general wellbeing. This additional training ensures the CSA's can recognise winter-related illnesses and issues, such as falls, cold homes, flu and social isolation during health and wellbeing checks for the over 65s. If the fitting of smoke alarms is recommended during a 'Safe and Well' visit, these will be supplied and fitted free of charge. If you are concerned about someone who you feel is at risk from a fire in their home, please call the advice line 0800 180 4140. Alternatively, you can use the online home fire safety check. The easy-to-follow home fire safety check has been developed through a partnership between the National Fire Chiefs Council (NFCC), Fire Kills and Safelincs. It will take you through your home one room at a time and the simple questions will help you spot fire risks as you go around your home. The tool will offer tips and advice on the steps you can take to reduce those risks. At the end, you will receive a personalised fire safety action plan to help keep you and your household safe from fire. |

The reasons for this are not clearly understood, but in some cases, blood tests in younger children are rarely helpful in guiding management and could cause the child and family increased distress. |

The team is made up of experienced paediatricians, a clinical psychologist, physiotherapist and occupational therapist offering support, advice psychoeducation and therapy for patients with persistent physical symptoms that are significantly affecting a young person’s daily life. We work together with children, young people, their parents, carers and education settings to understand and manage their symptoms, with the aim of improving both their day-to-day life. There is a Gloucestershire multidisciplinary team, able to share advice and guidance, or offer a rehabilitation approach, for those children and young people with more severe symptoms, impacting on a number of areas in their lives. It is suggested that you consider the 4 functional domains (school, hobbies & interests, managing every day and emotional responses) on the referral form when seeking either advice or referral. A paediatric dietitian develops nutritional guidelines for children based on their medical condition, usually after they are diagnosed with an acute or chronic illness. Paediatric dietitians teach the families about sound nutritional principles and the best ways to select and prepare food. In some cases, such as children who have kidney disorders, the diet modification may be complicated and require a great deal of guidance. Children’s Physiotherapy is a countywide specialist physiotherapy service for children and young people aged 0-16 years (16-19 in full time education), their families and carers, providing physiotherapy in both the hospital and community settings. We aim to provide an effective, safe, high quality service along with its partners and skilled workforce. The service provides assessment, advice and physiotherapy management of any childhood condition to promote the health and physical development of each individual. Treatment is planned around the child and families’ individual needs. The length of the treatment will vary for each child and condition. Children and young people are regularly re-assessed to evaluate progress and parents/carers are actively involved in the child’s physiotherapy programme. We also offer group work, including a lower limb class and hydrotherapy. The Children and Young People’s Physiotherapy service is provided in a variety of locations and settings. Access to physiotherapy is in the most appropriate setting for their assessment, treatment and ongoing support, enabling them to achieve their outcomes. This may be at a clinic, in a hospital, at a children’s centre, in a mainstream or special school or at home. The service operates from 8.30am to 4.30pm, Monday to Friday, excluding Bank Holidays, with some staff working flexibly to meet additional need. Some bases also offer evening clinics. We are trained nurses or midwives and work with public health nurses and community nursery nurses. We can help with infant feeding advice, referring on to other professionals after a full assessment, behaviour and sleep advice, continence as well as developmental assessments including speech, physiotherapy, hearing and eyesight. We can help with the early identification of social and communication difficulties, infant and maternal/paternal mental health and provide support with infant relationships, healthy families, diet/weight and exercise. We also provide baby hubs in each area which are drop-in sessions or appointments and provide relaxed settings for families to meet and chat with a member of the health visiting team and other parents and Carers for advice and support. They provide an opportunity to meet with a member of the Health Visiting team for advice and support. Infant Feeding The community midwife is there to support mothers with breastfeeding in the early days. Once the baby is 28 days old breastfeeding support would be given by the health visitor. The health visitors can be contacted directly or alternatively there are baby hubs across the county. Mothers can find the details in their child's 'Personal Child Health Record' (PCHR) or 'red book'. For more information please click here. The Home Safety Team work closely with health visitors, children’s centres and other child health professionals to make sure that Gloucestershire’s under-fives are well protected from unnecessary accidents. We provide a wide range of services designed to make a child's home as safe as possible.Our staff are trained to recognise potential dangers, and to give advice on measures to take to avoid serious accidents occurring. If there’s anything that we can do on the spot, we will. We can also offer advice on the right kind of safety equipment to use and if necessary we can provide and fit it. We send a copy of our survey shortly after our visit. School nurses are registered nurses or midwives and come from a wide range of backgrounds and experience in different areas of nursing - such as adult nursing, midwifery, mental health nursing, sick children's nursing or learning disabilities nursing. This gives school nurses a broad range of knowledge, skills and experience in order to help support your child or young person. School Nurses work with children and families who have concerns and issues associated with emotional and mental health, relationships and sexual health, bladder and bowel issues, lifestyle health and family worries. We can offer interventions, signposting and referrals to other services as required. Every secondary school in Gloucestershire has a school nurse drop-in where young people can visit their school nurse without an appointment. Discussions will be confidential and not shared with schools or parents if that is the young person’s this request. To find out when and where in school the drop-in is taking place the young person should ask someone in school reception or their year head. School nursing hubs provide an opportunity for parents to meet other parents with similar worries and concerns about their children and engage in a facilitated discussion which will include top tips, advice and support. Topic areas discussed will be about their child's challenging behaviour which may include sleep or healthy weight and oral health. The WellChild Nurse is focused on planning and coordinating the child/young person’s transition from hospital to home. TIC+ offers evidence-based counselling for children, young people and families with mild to moderate mental health needs. TIC+ supports young people struggling with issues such as depression, anxiety, anger and grief, as well as offering support and advice to parents and professionals. They work to ensure that every young person requesting support has prompt, independent access to the service. Their face-to-face counsellors will travel to see the young person at their school, college, or near to where they live, making the counselling far more accessible for the young person who may not be able to travel to a central location.

The specialist Paediatric Bladder and Bowel Care Service is delivered by Paediatric Specialist Nurses and a Specialist Nursery Nurse. The Team all have a special interest in bladder and bowel issues in children and young people. We have undergone additional training in aspects of bladder and bowel care. The Specialist Nurses provide an assessment and treatment plans, with ongoing evaluation, advice and support over 8 sessions. After this, a plan to discharge or refer onwards will be formulated, with the parents/carers/child or young person. Some children, with complex issues or, who are provided with continence product prescriptions will remain under the care of the team until they reach the age of 18 years, before transition to relevant adult teams. The service works in partnership with all other GHNHSFT services and the following external providers: Gloucestershire Health and Care services, universal health (GPs, Health Visitors, and Public Health nurses), Education providers, Social Care and family support services and the voluntary sector. The service aims to improve the clinical outcomes and quality of life for children and young people and their families through evidence-based treatment that promotes continence, preventing unnecessary long-term reliance on nappies and pads and the possible need for surgery The Paediatric Bladder and Bowel Care Service offers a wide range of interventions ranging from tips and techniques to motivate children and develop their skills, to drug treatments and bedwetting alarms as part of a more complex and comprehensive treatment plan. The team provide on-going support and products tailored to the needs of children and young people. Appointments are scheduled for a review of progress at least every 6 months. |

The service is a community based service, provided by GHC, designed to manage patients with complex leg wounds. Patients over the age of 18 who see their GP with the presentations below should be referred in to this service for a full holistic assessment which will determine which pathway the patient follows:
Patients determined to fall within the ‘simple pathway’ will be referred back to the GP in a compression hosiery kit, where appropriate, and with a treatment plan for the Practice Nurse to follow. Complex patients will remain in the service for treatment. Other referrals may be deemed to be appropriate during the initial assessment such as to dermatology or the vascular team. The service aims to work with the patient to promote wound healing, utilising specialist vascular, orthopaedic and dermatology colleagues in the hospital as necessary and requesting venous scanning as part of assessment. They enable people to achieve self-management wherever possible and to provide education and supervision to health care professionals engaged in providing care for those living with leg wounds. |

A number of GP Practices in Gloucestershire have signed up to an enhanced service to provide minor skin surgery for Gloucestershire patients who are not registered with their Practice. GPs in Gloucestershire can refer patients on to these practices for surgery if they are unable to offer the patient treatment within their own Practice. This service provides an alternative to referring these patients to the hospital surgery, allowing patients to be treated within primary care and reducing pressure on the local dermatology service. Treatment offered under this arrangement is in line with the national Minor Surgery Direct Enhanced Service, however there is some variation in the types of lesions treated at each Practice based on local skills and experience. Botulinum Toxin for the Treatment of Hyperhidrosis - Prior Approval The ICB will provide funding for Botulinum toxin for hyperhidrosis for patients who meet the criteria defined within this policy. Funding approval for eligible patients must be sought from the ICB via the Prior Approval process prior to initial treatment. Where treatment is successful there is no requirement to seek Prior Approval for subsequent injections where the treating clinician believes this will continue to deliver benefit. Removal of Benign Skin Lesions - Prior Approval Treatment for a range of asymptomatic skin lesions is considered a low priority for funding and will only be considered in exceptional circumstances. Funding approval must be sought from the ICB via the Individual Funding Request process prior to treatment. Removal of symptomatic skin lesions in primary or secondary care is funded for patients that meet the criteria set out in this policy. Funding approval for eligible patients must be sought from the ICB via the Prior Approval process prior to treatment. Where removal of a lesion is supported the ICB expects that removal will be undertaken within Primary Care wherever possible through the Minor Surgery Direct Enhanced Service/Inter-practice Minor Surgery Enhanced Service. Referrals to secondary care for surgery to remove skin lesions must therefore be clear about why the patient is not suitable for surgery within Primary Care. |

Screening for diabetic retinopathy should occur soon after diagnosis and then when invited by the Screening Programme to allow early detection and management of any problems. For children over the age of 12 annual screening should be arranged by the GP. People eligible for diabetic eye screening who have had two consecutive eye screens showing no signs of diabetic retinopathy will be offered screening every 2 years as it is safe to do so, with some exceptions for patients we have referred into the hospital eye services. The NHS Diabetes Prevention Programme (NDPP) is a joint initiative between NHS England, Public Health England and Diabetes UK, which identifies people who are at high risk of developing type 2 diabetes. It offers them a free lifestyle intervention encompassing group educational sessions in line with an evidence-based specification, in order to reduce their weight and increase physical activity, thus reducing their risk of developing Type 2 Diabetes. Provider & Service Offer: Gloucestershire’s NDPP service is delivered by Xyla Health & Wellbeing. Xyla H&W have now secured the contract to continue providing the NDPP programme in Gloucestershire until 2025. Referring patients into the NDPP now will provide the opportunity for patients to access:

*Individuals who satisfy all of the following eligibility criteria should be referred to the NDPP Service All patients with NDH are eligible for the NDPP service unless they meet any of the exclusion criteria below. Patients aged over 80 years NHS Diabetes Prevention Programme (NDPP) Expert Reference Group have reviewed the data for the programme of those over the age of 80 and have found that the overall benefit of lifestyle programmes directed at diabetes prevention are less robust for these individuals. Therefore, is has been recommended that an individualised approach should be adopted when referring people aged over 80, with clinical consideration of the risks and benefits for the individual prior to referral. In addition, people recorded as having moderate or severe frailty should not be referred to the NDPP. This means that mailout of NDPP initiation letter should not be used for people aged over 80 without prior consideration for the risks and benefits of a programme. Gestational Diabetes Mellitus - eligibility Patients that fall under this criterion currently can only be referred directly to Xyla H&W following receipt of informed consent and therefore cannot be invited to self-refer. The NDPP referral form has been updated on Arden’s templates to highlight the patient has a history of GDM, also a patient information leaflet specifically for women who’ve had GDM has been created for you to share. NHS England has provided a briefing and FAQ document to support HCPs with referring these patients. Exclusion criteria:

Advice from a Consultant Radiologist will be available if clinically appropriate, our Consultants have specialist interests, which include Musculoskeletal, Gynaecological, and Vascular Ultrasound. Our ENT service cares for patients with a range of routine and urgent conditions. As well as providing a general ENT service, we also offer specialist services that treat deafness and ear disease, complex sinus problems, head and neck cancer and voice problems. Many of these conditions are managed by multi-disciplinary teams (MDTs) where ENT staff work closely with other health professionals such as pathologists, speech therapists, specialist nurses, audiologists and hearing therapists. Hospice at Home:Our Hospice at Home staff might do up to 3 visits per day to each patient, and overnight sits. They’re there to support the patient’s needs but also their family members. The service gives people the choice to die in their own homes if they wish to do so, surrounded by family and carers, pets too, or on their own if this is their choice. It often prevents unnecessary hospital admissions and enables people to be discharged from hospital to die. Clinic times vary slightly. You should allow 2 hours for your appointment, as emergencies may cause delays. The appointment includes assessment and or investigations e.g. pregnancy tests +/- ultrasound +/- blood test/s depending upon the clinical need. The EPA team has developed the referral form to ensure that patients with pain and bleeding in early pregnancy or patients suspected to be at risk of ectopic pregnancy are triaged and offered a suitable appointment according to clinical need. All GP referrals to the Early Pregnancy Service at GHNHSFT should be made by email to: The team have developed this referral form to ensure they have all the relevant information to clinically triage the patients. GPs are asked to please complete the form and select the appropriate category (shown below) and then send via urgent email to ghn-tr.earlypregnancyreferrals@nhs.net:

We are a referral only service (via our website) and so only have direct contact with professional workers, such as Community Family Workers and Health Visitors. If you are supporting a refugee / asylum seeker family residing in one of the hotels, the referral needs to come via GARAS. Once the referral is received, a volunteer will check what we have available and put the ‘bundle’ together. If we do not have the needed item/s the referral will be put on a waiting list. Once contacted, it is then the referrer’s responsibility to collect and pass on the ‘bundle’ to the pregnant woman, parent or family. |

The consortium is made up of Home-Start North and West Gloucestershire, Home-Start Cotswolds and Home-Start Stroud and Gloucester. Between these three Home-Starts, families throughout the county are able to access help and support. Gloucestershire Pregnancy Advisory Service offers advice and support for pregnant women who wish to explore their options if they are undecided or unable to continue with the pregnancy. The service is commissioned to provide terminations within county up to 16 weeks gestation. Medical terminations take place onsite at Hope House and Surgical Terminations are carried out at GHNHSFT. After this date (up to the legal limit of 23 weeks) referrals will be seen and treated by another provider i.e. British Pregnancy Advisory Service (BPAS) or Marie Stopes. The service accepts GP, Practice Nurse, School Nurse (or any other professional) and self-referrals. Where the referring doctor has not provided a signed HAS the service will make the necessary arrangements and bear overall responsibility for the completion of the forms required under the Abortion Act. All necessary investigations will be performed in the clinic. |

Our Colorectal Surgery Service provides a full range of diagnostic tests, scans, outpatient consultations and treatments for patients with symptoms suggestive of benign and malignant diseases of the gastrointestinal tract including colorectal cancer, inflammatory bowel disease and anorectal conditions such as haemorrhoids. Our service also investigates patients referred on the suspected colorectal cancer (two week wait) pathway. Our service treats benign bowel disorders including diverticular disease, inflammatory bowel disease, haemorrhoids and fissure. We also treat pelvic floor disorders, as well as colorectal and anal cancer. |

The IBD Team at GHFT is made up of consultant gastroenterologists, surgeons, registrars (doctors close to the end of their speciality training) IBD specialist nurses, a specialist pharmacist and dieticians. The team works closely together and hold regular meetings where complex cases and patient progress are reviewed. The team can be contacted via the IBD Advice line or email below where patients / GPs are able to leave messages with queries relating to IBD conditions and medications / treatments. Messages are picked up throughout the day Monday – Friday and the queries are then dealt with accordingly. All referrals to Hepatology, including for Fibroscan, at Gloucestershire Hospitals NHS Foundation Trust (GHNHSFT) should now be sent via the ‘Hepatology Triage Service’ on e-RS rather than being referred to individual clinics. Once received by GHNHSFT, referrals will be assessed by a consultant who will make a decision on the most suitable onward pathway for each patient based upon the referral information provided. Depending upon the triage outcome, patients will either be directed straight to Fibroscan, offered an outpatient appointment or the referral will be returned to primary care with management advice.

North Bristol NHS Trust has been designated as one of nine Specialised NHS England Complex Mesh Centres, supporting patients with mesh complications linked to urinary incontinence and vaginal prolapse in the South West. These centres provide management of urinary incontinence and vaginal prolapse mesh complications, with the engagement of the multi-disciplinary team (MDT), which includes surgeons, physicians, imaging specialists, nurses, pain specialists, physiotherapists, and clinical psychologists in line with the published service specification. Referrals will be triaged by the Clinical Lead(s) and invited to an outpatient appointment. They will be referred to selected members of the MDT as required, and each patient will be discussed at the monthly MDT meeting. The Nelson Trust Women's Centres in Gloucester, Swindon and Bridgwater provide a women-only space where a wide range of support needs can be addressed in a safe and supportive environment. Services are trauma-informed and gender responsive providing holistic support for women and their families. They accept referrals from any professional or women may refer themselves. Every woman is offered a detailed individual assessment of their needs across nine “pathways”:
Each client has their own Keyworker who works with them to co-create a support plan to address their needs. This will be achieved through one-to-one sessions, group work and access to specialist services across the county. They run group sessions including Beyond Anger, Parenting groups, Relapse Prevention, Pattern Changing and Mentoring. The instructors have attended specific Cancer Rehab training so are able to facilitate exercise sessions which can aid your cancer journey wherever you may be in it. Cancer exercise is suitable for people who might have just been diagnosed, going through treatment or have just finished treatment. You need to be over 18 to attend the classes and they are suitable for any level of fitness. In Gloucestershire we have a wide range of exercise options, such as yoga, tai chi, and gym sessions available at facilities across the county including gyms and sports centres. |

New technologies have a significant part to play in providing flexibility and empowerment to patients to take control of their care through supported self-management and the Personalised Care agenda within the NHS Long Term Plan. Physical Activity is acknowledged as possibly the single most important thing to do for people living with a long term condition. To date however, Physical Activity interventions have been largely unsuccessful in delivering patient benefits due to the emphasis on sport and structured exercise. This approach has failed to engage those most at risk and to limit access for people managing chronic disease. KiActiv® Health overcomes these issues with a personalised service that is proven to be appealing and accessible to a diverse population with no barriers to age or mobility and delivers measurable and sustainable behaviour change. The programme has been refreshed for the winter and is offered countywide but we are now keen for your clinical team to signpost patients to this free, personalised 12-week programme, enjoyed from the comfort of their own home. The KiActiv® Health online service integrates the latest science and technology with remote mentor support for 12 weeks to empower patients to increase their everyday physical activity in the context of their condition. It demonstrates to the patient that every move they make can count to improving their health without compulsion or prescription. An advice service for GPs, which covers older people’s mental health problems such as dementia, depression and psychosis. The service offers clinical advice, with a view to the service being more responsive. It is not intended to be an urgent referral route nor to replace phone calls to local consultants. An email inbox will be manned by a specialist doctor (consultant, staff grade or specialist registrar) Monday – Friday. Replies will be made by phone or email, in line with callers’ preferences. If the patient ID can be given in the email, the doctor can consult clinical systems before calling back. The service is aware that surgery switchboard numbers can be extremely busy. GPs who would prefer to be phoned directly should leave the number on which they would like to be contacted. |

The Gloucestershire Recovery in Psychosis team or “GRiP” is an Early Intervention in Psychosis team that work with people aged between 14 and 65 who are at risk of experiencing (at risk mental state or ARMS) or who are experiencing a first episode of psychosis Typically, Early Intervention teams work in the community and will try to meet with people in a place where they feel most comfortable providing self-help information, or signposting to another more appropriate service to help with their presenting difficulties. Patients deemed ARMS will receive more targeted support from the Early Intervention Service. As part of the national role out of the Community Mental Health Transformation Programme, in Gloucestershire we have co-designed and co-produced a new model for the way in which we support people with serious and enduring mental illness – Locality Community Partnerships (LCPs). LCPs are regularly held (either weekly or fortnightly) virtual meetings, where members from health, social care and VCSE partners come together in a multidisciplinary approach, to discuss how best to support someone with a serious or enduring mental illness, who do not currently fit a single service or may have a level of complexity that may need a multi-agency offer. There are a range of mental health services people can be referred to, nearly all having acceptance and exclusion criteria. When people do not meet the criteria, they cannot access services and where people meet the criteria the service takes on almost sole responsibility of needs even when they are complex. The LCP brings a ‘no wrong door’ approach, supporting clinicians, professionals, and service users in navigating access to community, specialist MH and VCSE services and supports the development of a collaborative, personalised support offer. The Gloucestershire Specialist Perinatal Mental Health Team prioritise women with serious and complex mental health needs, including those who have experienced trauma and loss. Provides early identification and treatment for women/birthing people with moderate to severe mental health difficulties, including birth related trauma and loss. Support provided to the whole family and the developing relationship with the infant. Preconception advice to women with a history of serious mental health difficulties considering pregnancy is also offered. We are a multidisciplinary team made up of a consultant perinatal psychiatrist, specialist nurses, clinical psychologist / therapists, occupational therapist, social workers, nursery-nurses, specialist midwives and peer support workers. The Birth Anxiety and Trauma Service is a part of the Perinatal Mental Health Team and we provide dedicated psychological support for women living through birth trauma, fear of birth or pregnancy and baby loss and are experiencing high levels of distress in relation to their maternity experience This service is NOT AN EMERGENCY SERVICE. The Community Bone Health Service is a nurse-led service which help patients identified as having had at least one low impact fracture with an aim to prevent secondary fracture. Patients will be assessed for risk factors for osteoporosis. This will include blood tests to exclude secondary causes, which in some instances will identify incidental findings/ abnormalities which may require primary care involvement. Assessment will include referral for bone density scan if indicated and commencement of treatment as per local and national guidance. All communication, with treatment and management plans, will be sent to the General Practitioner. People identified as benefitting from this service would be predominantly in middle & older age-groups. Specifically, in order to adhere to National guidance, people younger than 50 years, or pre-menopausal women should be referred to a specialist service. |

Acute MSK presentations are usually described as within 6 weeks of onset of symptoms or injury (e.g. a suspected recent fracture or a serious soft tissue injury (eg. unable to weight bear or use the limb). Changes to the process will ensure an orthopaedic consultant review of all fractures electronially (without patient present). These referrals are reviewed 7 days a week by the Trauma and Orthopaedic surgeons; the vast majority are reviewed within 24 hrs and all are within 48 hrs. Patients will be contacted by phone either to arrange a face to face clinic appointment or to discuss their injury and further management. |

The Brain Injury Team is a multidisciplinary therapy team (i.e. no medical input) based at Gloucestershire Royal Hospital. They offer in and outpatient assessment and therapy for adults (16 years+) who have had an acquired brain injury and who are a resident in Gloucestershire with a Gloucestershire GP. The team offers intensive acute therapy immediately post injury, through community rehabilitation and outpatient follow up, sometimes years after the brain injury. They cover the spectrum of mild to severe brain injuries. The team is based at Gloucestershire Royal Hospital but is a countywide service. The team is able to offer specialist assessment of patients in possible low arousal states using the Sensory Modality Assessment and Rehabilitation Technique (SMART) assessment tool. The team provides outpatient services in patient’s homes, residential accommodations, nursing homes etc. The specific therapies within the team are detailed below. The therapists are supported by Brain Injury Therapy Technicians. Mild Head Injury Clinic This clinic is run by the Brain Injury Team and is suitable for everyone who attends the Emergency Department or has a brief inpatient stay following a head injury, and has residual problems. The aim is to offer them at least one appointment at the ‘mild head injury clinic’ within a few weeks of their discharge. GPs can also refer directly to this clinic for patients who have had a mild head injury. This may be associated with a brief loss of consciousness and/or a period of time for which the patient has no recollection. Scans may have detected no abnormalities as yet. In addition to the mild head injury clinic providing follow-up for patients who attend hospital, it can be a useful first contact for people even years after their head injury, as it can provide a brief assessment and inform subsequent referrals to appropriate head injury specialists when required. The mild head injury clinic is run by the GHNHSFT Brain Injury Team’s clinical neuropsychologist. There is also a clinic run by the physiotherapist and the psychologist. Patients are seen by the physiotherapist to address difficulties primarily with balance and dizziness. A common consequence of a brain injury is dizziness, feelings of vertigo and poor balance. The team’s physiotherapist runs a clinic specialising in the identification of the source of the symptoms and the development of treatment programs to reduce patient’s difficulties (vestibular rehabilitation). Dizziness may also be assessed as part of a wider brain injury assessment in specific physiotherapy sessions. |

Community Dementia NursesCommunity Dementia Nurses provide support to GP practices in the diagnosis, management and treatment of dementia. The service also provides short-term support to people with dementia and their carers to address issues and immediate care needs arising from their diagnosis. This service can be accessed through the patient’s or carer’s GP using the referral form linked below emailed to Cdn-Inbox@ghc.nhs.uk. Community Dementia Nurses work closely with the Dementia Adviser Service that is delivered through the Alzheimer’s Society. Dementia Advisers help anyone diagnosed with dementia and their families to get the information they need about the illness and where to find the most appropriate services in their local area. The Mindset course is provided for a group of around six to eight people who have recently (within 12 months) been diagnosed with a form of dementia. It is an ideal way to provide emotional and practical support and can often help them to adjust to living with the illness. The courses are free to attend and accept both clinical and self-referrals. There is a leaflet explaining the course that can be given to patients. Mindset courses provide both practical information about living with dementia alongside a supportive environment for people to share and express their concerns about their diagnosis. The course runs over eight weeks and is only open to people living with dementia. However, there are additional sessions before the course starts and after it ends, that family members and friends are invited to attend. Topics covered include practical tips such as how to use memory aids and keep active as well as support to manage emotions including stress and worry. This course is run by Mindsong, a Gloucestershire Charity with 10 years of experience working with dementia. The charity works closely with NHS Gloucestershire, Managing Memory, Alzheimer’s Society, Community Dementia Nurses and local Frailty Teams.
Sessions last for 90 mins and are run in an accessible venue by an HCPC registered Music Therapist (although music may or may not be used) with a member of our team in support. Following referral, a therapist contacts the person to explain the course, to assess its suitability, and to get their consent. Each referral is assessed on an individual basis, but our main criteria are:
Mindset is strongly based on Living Well with Dementia (LivDem), developed by Professor Richard Cheston and Consultant Clinical Psychologist Ann Marshall (livdem.co.uk). We offer a full complement of bariatric procedures and a comprehensive multidisciplinary approach. The team comprises surgeons, bariatric aestheticists, consultant obesity physician/endocrinologist, specialist dietitians, nurse specialists, psychologists and our bariatric coordinator. Bariatric surgery is commissioned for patients identified by the Tier 3 Specialist Weight Management Service who it considers may benefit from surgical intervention. Bariatric surgery is not a cure for obesity or a safe and suitable option for all patients. Note that surgery may also be unsuitable (or delayed) for patients unable to consistently demonstrate all required behaviours, safely and appropriately, across a range of circumstances. For example, conditions where mental health may fluctuate or de-stabilise with risk or crisis episodes or in the presence of difficult life circumstances – e.g. significant bereavement or new physical or mental health episode or diagnosis (e.g. new cancer diagnosis). |

NHS England and Improvement have procured digital weight management services from a number of providers through the NHS Digital Weight Management Programme. Emerging evidence from the NHS Diabetes Prevention Programme suggests that some digital providers can perform as well as face to face services, with benefits in reaching a younger cohort and offering increased flexibility of access. The Specialist Weight Management Service (SWMS) is a service provided by Gloucestershire Hospitals NHS Foundation Trust and is aimed at improving and promoting weight loss and quality of life. The service is psychologist led including specialist dietitians and provides the level of support needed to allow patients to lose the weight needed to improve their health and also to maintain the weight loss. Some patients might also be referred to surgical team for assessment of suitability for bariatric surgery. Referral for Tier 3 Specialist Weight Management is a pre-requisite of NHS funded bariatric surgery. However, the primary function of this service is to offer non-surgical weight management treatment. The CUES provides urgent assessment, treatment or referral for sudden onset eye problems such as flashes, floaters, vision loss or minor eye injuries. Who can be referred to CUES: This service is free and available to people of any age, but children must be accompanied by an adult, and they must be registered with a local GP in Gloucestershire. How to refer to CUES: This is a self-referral service so please provide the patient with a CUES service leaflet and signpost them to ‘Find a Local Practice’ tool. Advise the patient to choose an Optical Practice from the list and contact them as soon as possible, same day preferably and ask for an urgent eyecare service appointment. *Please note - if the patient is unable to contact CUES themselves, please refer on their behalf. The Optical Practice will take the patient's details and an appointment will be given within 24hours/5 days depending on the severity of the patient's symptoms/history. The patient should take the referral letter along to the appointment. The GP will receive a report from the Optometrist on the outcome. Key Patient information: Patients referred into either pathway should expect to undergo dilated fundoscopy. Dilating eye drops will be instilled in both eyes. Their pupils will remain dilated for around four hours after the examination, and they may experience blurred vision. Therefore, they should be advised not to drive themselves to or from their appointment and they should not drive until the effects of the drops have worn off. They may also be light sensitive. |

The Community Dental Service comprises the Special Care and Paediatric Referral Service, Intermediate Minor Oral Surgery, General Dental Service and Out of Hours urgent Gloucestershire dental care. The Community Dental Service provides NHS dental care for people in Gloucestershire who are unable to access treatment from a general dental practitioner. It provides routine dental healthcare check ups, emergency appointments and out of hours emergency dental pain relief. Most of the dental clinics are fully accessible. They also provide special care dentistry for those who are unable to access routine dental care due to mobility issues or specific learning needs or those who have a high level of anxiety over dental care. Domiciliary care can also be provided to those whose circumstances make it difficult to leave their home. We help people of all ages to manage pain which has been present for more than 3 months, although many of our patients will have had pain for much longer than this. Please explain to your patients that the service will review their referral and they will be assessed by the appropriate member(s) of the team, which might not be a Pain Consultant. The service is now offering telephone and online talking interventions including online group programmes. Relevant professions in the pain service include pain-specific psychologists, physiotherapists, nurses, and consultants in pain medicine. The consultants are specialists in symptom management, not diagnosis and are available to consider particularly complex medication issues or to advise on the medical management of pain from another cause where this co-exists with fibromyalgia. Specialist Nurses can provide support for medication optimisation and reduction if needed. Other pain management services include assessment of pain, education, advice and reassurance, pain management programmes, mindfulness courses, 1:1 CBT and specialist exercise interventions for those who have complex requirements preventing access to primary care offers. Some people with particularly complex pain presentations may benefit from referral to the tertiary pain management service at the Bath Centre for Pain Services www.bathcentreforpainservices.nhs.uk. In order to access funding from NHS England, referrals must be sent from secondary care services demonstrating that all local options have been exhausted or are inappropriate. Chronic Pain MDT provided by GHNHSFT includes Physiotherapy, Occupational Therapy, Nursing, Psychology and Consultants in Pain Medicine. Services include assessment of pain, education, advice and reassurance. Specialist Nurse can provide support for medication reduction if needed. GRS is an integrated specialist community respiratory service delivered by nurses, physiotherapists, occupational therapists, clinical support workers and administrators in the acute sites (GHFT) and the community (GHC). The service strives to achieve a seamless system-wide pathway of care for patients with chronic respiratory disease and works closely with Gloucestershire respiratory medicine consultants. The community respiratory team provides clinic appointments (or home-visits ONLY for house-bound patients) to optimise the management and quality of life of patients with chronic respiratory disease including COPD, asthma, bronchiectasis and interstitial lung disease. Home Oxygen Assessment ServiceHOAS offers specialist review for those people who are receiving home oxygen therapy or who may meet the criteria for assessment. The team offer clinics around the county usually in community hospitals and home visits may be offered to those who are house bound. Please note we do not accept referrals for those who are still smoking. Air Liquide supplies home oxygen across the South West region. In cases where a patient has been prescribed oxygen via a HOOF A, the prescriber MUST complete an IHORM/HOCF for risk mitigation and patient consent. The HOOFA (prescription) can be found on the Air Liquide website https://www.airliquidehomehealth.co.uk/hcp/ and the IHORM/HOCF is available here to upload to your clinical system. All patients prescribed oxygen under a HOOF A must also be referred to the HOAS team via the referral form. GPs are now only able to prescribe an urgent concentrator (which will come with a backup cylinder) for patients who are considered end of life. The Early Inflammatory Arthritis (EIA) Clinics at Gloucestershire Hospitals NHS Foundation Trust have been running since 2nd April 2013. Their focus is early diagnosis and treatment of patients referred with suspected inflammatory arthritis, aiming to start treatment within three months of onset of symptoms. Research evidence shows that early treatment of inflammatory arthritis leads to better disease control, less joint damage and disability and improved long-term prognosis. The Early Inflammatory Arthritis (EIA) Clinics are held weekly on Tuesday mornings at Cheltenham General Hospital and on Tuesday afternoons at Gloucestershire Royal Hospital. Patients who meet the referral guidelines can be referred to the Early Inflammatory Arthritis (EIA) Clinics on the NHS e-Referrals Service. We aim to see patients within three weeks of receipt of referral. Please complete the referral proforma which will help identify patients with suspected EIA and guide initial investigations. Please note that a summary letter outlining the patient's history, joints affected, analgesics and anti-inflammatories used together with previous medical history is also required. Gloucestershire ICB supports the use of the National Early Warning Score (NEWS) as a tool to help establish a baseline of a patient’s condition and thus improve communication between services caring for a patient in an emergency situation. South West Ambulance Service (SWAST) is already calculating the NEWS for patients and providing it at arrival in the Emergency Department. When you contact the Single Point of Clinical Access (SPCA) to advise on your emergency admission they will be requesting the NEWS from you. We ask that GPs use the attached transfer form (incorporating the NEWS), which will improve communication between services and assist SWAST and the Emergency Department with triage. This will help to identify and prioritise patients of highest risk of deterioration: appropriate interventions will then be targeted on patients at highest risk. |

The Single Point of Clinical Access (SPCA) is a dedicated service that supports clinical professionals to arrange the right care for community referrals and bed based transfers, helping to prevent hospital admissions and facilitate the management of patients within the community. The SPCA is a team of senior nurses managed by Gloucestershire Health and Care NHSFT (GHC). The SPCA can assist in signposting to other services if more appropriate to do so such as:
SPCA can support admissions to non-acute bedded options such as Community hospitals and the Community Assessment and Treatment Unit. The SPCA service (0300 421 0300) is open between the hours of 8.00am – 10.00pm, Monday to Friday. The opening hours for weekends and Bank Holidays are 9.00am – 5.00pm. The Gloucestershire Leg Ulcer Service is a countywide service, it is a vascular led service dedicated to the evidence-based treatment of leg ulcers. Leg ulcers are not wounds but are generally symptoms of an underlying vascular disease. Our team of specialist vascular professionals provide a full range of vascular care for our patients in a range of vascular diseases. Local healthcare professionals can access specialist vascular advice via the CINAPSIS smart referral service during on call hours. Team for Hearing Impairment
Qualified Specialist Advisory Teachers of the Deaf work with children and young people from birth to 25 years with a diagnosed hearing loss.
Team for Visual Impairment
works with children with a significant level of visual impairment. Children who are visually impaired vary widely in their educational needs. Therefore provision reflects the individual needs of visually impaired young people.
Referral to the Advisory Teaching Service should come from a health professional, the school, Early Years setting or SENCo. Complete the "Request for Advisory Teacher Involvement" form available online.

A child will need to have been seen by an Ophthalmologist before the Advisory Teaching Service is involved. GPs can refer to an ophthalmologist following an assessment by an optometrist.

Our Children in Care Health Service Team provides health support for all children and young people that are placed in the care of Gloucestershire County Council.
Our aim is to help children and young people with their physical and emotional health needs and promote healthy lifestyle choices.
Our nurses are all registered nurses as well as qualified health visitors or school nurses who have also had additional training around the needs of children in care.
We work closely with social care and the looked after children’s services including foster care services, within the local authority.
Anyone involved in supporting children in care can contact us for advice and support, including health professionals, social workers, foster carers and public health nurses.
Our aim is to promote and look after the health of children in care. We do this by:
The team are available for advice as requested from any service/individual as well as providing support to social care teams. We work closely with social care and the looked after children’s services including foster care services, within the local authority. Anyone involved in supporting children in care can contact us, including health professionals, social workers, foster carers and public health nurses.
Early help is about providing support to potentially vulnerable children, young people and their families as soon as problems begin to emerge or when there is a strong likelihood that problems will start in the future.
The purpose is to prevent issues and problems becoming serious and harmful to the child, young person, family and community. When children and families need this extra support, they need it quickly. Provision of early help support can be at any stage of a child's life; pre-birth, during pregnancy, childhood or adolescence.
Who provides Early Help in Gloucestershire?
The Early Help offer within Gloucestershire is provided by a number of services including Schools, pre-school settings, children’s centres, health care providers, school nurses, health visitors, youth supports services and Families First Plus alongside services provided by the community and voluntary sector.
A range of information and support is available to both families and professionals working with them through the glosfamilies directory.
How to request support from Early Help?
All services have their own referral route, and if you know what service is required you are able to refer direct. However, where you are unclear about what support is needed or could be provided, you can utilise a fortnightly allocation meeting held in each locality. These meetings are co-ordinated/administrated by the Families First Plus teams, however the support offered to a family will be bespoke to the needs highlighted. The meeting has representation from schools, housing, police, social care, youth support, health and voluntary sector amongst others. A family maybe offered support from a variety of those services represented depending on what is felt to be the right action at that time. The meetings are used to share information and explore support options available to families depending on the needs highlighted through the service request form. The referrer, this could be a school nurse or GP or any other professional, may also be invited to attend the meeting and contribute to the discussion.
To have your case discussed at the allocation group professionals will need to complete a Request for Support Form. As Early Help services are not a statutory service, professionals will need to obtain consent from the family and complete the Consent Form with them prior to referral.
If at any time in early help work, it is considered that the child is at risk of significant harm, practitioners must follow the local safeguarding procedures and contact the Children’s Helpdesk on 01452 426565. Unless a child or young person is at immediate risk of significant harm, the practitioner completes a Multi Agency Request Form.
Presenting issues:
The Early Years Service brings together the statutory and non-statutory duties of the Local Authority. It combines the functions previously undertaken by EY Locality Advisers, pre-school workers, EY finance, EY commissioners and the EY SEND service.
The aim of the service is to improve outcomes and reduce inequalities for all children aged 0-5 years, by:
The Early Years Service continue to accept requests for support for children who are not yet accessing, or able to access, an Early Years Provision. We will continue to work with the child and family in the home environment and also look to support them in identifying an Early Years Provision or community group to access, should parents require this.
Please use the ‘Early Years Service – request for support – child at home (DOCX, 75.5 KB) ’ to make any referrals to the Early Years Service, and ensure that you include as much information as possible so that we are able to process your request accordingly i.e. My Plans, ASQ assessments.
Please note: - If you are concerned about a child’s progress or inclusion in an Early Years Setting, please discuss this with the setting first, and ask them to make a referral to us using the appropriate form – ‘Early Years Service – setting support request form (DOCX, 67.5 KB) ’ – we would ask that the setting complete and submit this request if required.

Gloucestershire Educational Psychology Service provides psychological advice and support through a consultation model for schools, families and children. We work with schools to support children’s learning, development and emotional well-being.
Educational psychologists work at all levels within educational settings: supporting whole school projects, staff training and coaching, group and individual support work with children, as well as the assessment of the needs of children with SEND. We work closely with other Local Authority support services including the Advisory Teaching Service. We have close links with professionals working in Social Care, and Health.
Each maintained school and academy in Gloucestershire has an allocated EP and schools are invited to take out a subscription with our traded service every academic year to complete a range of work within their setting.
A core element of our work is in providing Psychological Advice as part of the Education, Health and Care Needs Assessment process for the Local Authority. For every statutory assessment the Local Authority has a duty to request Psychological Advice from one of our Local Authority Educational Psychologists.
