Integrated Discharge Team

The Integrated Discharge Team is made up of a group of professionals from both social care and health care at both the Gloucestershire Royal Hospital and Cheltenham General Hospital sites. 

The Integrated Discharge Team (IDT) work to achieve two outcomes:

  1. helping people stay out of hospital by directing them to other services in the community which will mean they don’t need to be admitted to a ward
  2. supporting patients who are in hospital and who will need ongoing health or social care services after they are discharged.

Patients, carers and relatives may come across a member of the Integrated Discharge Team either in the Emergency Department, the Acute Care Units or on one of the wards. The Integrated Discharge Team work in close partnership with other clinical staff from the wards and departments.
