Preparing, planning and moving from children’s to adult services

The transition from children's to adult services is a gradual process and should prepare the young person for the self-management of their condition

We have started the transition pathway, called 'Ready, Steady Go,' within the specialities of epilepsy, renal, diabetes and rheumatology.

Who is it for?

The process of the transition can start from the age of 11 years old for all children and young people who have a long term condition such as diabetes or epilepsy.

What is it?

Ready, Steady Go is a transition programme to adult care that will help your child gain skills and knowledge to help them manage their condition.

Why?

The programme helps to prepare your child in the move to adult services when the time is right.
