Screening for Suicidal Ideation - for Professionals

The Columbia-Suicide Severity Rating Scale is well validated and, for adolescent patients, scales such as The Tool for Assessment of Suicide Risk for Adolescents (TASR-A), may be of use.

In a 2012 study by Horowitz and colleagues looked at validation of a simple four-point questionnaire :

1. In the past few weeks, have you felt that you or your family would be better off if you were dead?

2. In the past few weeks, have you wished you were dead?

3. In the past week, have you been having thoughts about killing yourself?

4. Have you ever tried to kill yourself?
