Media Consent
Agreement is given to appear in photographs/video/radio recordings to be taken for Gloucestershire Hospitals NHS Foundation Trust for publicity, information, education and exhibition purposes. I understand that they may be used in printed articles seen by the general public, social media posts, on the internet on radio, television or in medical textbooks on public sale. I release Gloucestershire Hospitals NHS Foundation Trust and/ or its appointed agents from any liability resulting from the taking, producing or completing the photographs and/ or video/radio/webcam applications and/ or being quoted/ misquoted following an interview by media or hospital personnel. I understand I will not receive monetary or monetary compensation for using my name, words, recording, photographs and/ or video applications. I understand that on some occasions, the copyright for images and/ or footage produced by a third party may remain with that third party rather than with Gloucestershire Hospitals. Consent must be freely given (i.e. not coerced) and can be withdrawn at any time.
